Qualifications

• Active West Virginia RN license required.
• Strong clinical assessment and critical-thinking capabilities.
• Excellent communication with residents, families, and colleagues.

Responsibilities

• Assess residents and monitor changes in condition and vital signs.
• Administer medications and treatments per physician orders.
• Develop and update individualized care plans with the care team.
• Communicate changes in resident condition to physicians and leadership.
• Educate residents and families on care and health management.
• Maintain accurate clinical documentation and coordinate with teams.
• Follow infection control and safety standards.
• Provide compassionate, resident-centered care respecting privacy and preferences.
• Demonstrate sound clinical judgment and respond to needs promptly.
