Responsibilities

• Performs nursing functions based on the Nursing Process: Assessment, Diagnosis and Planning, Implementation, and Evaluation.
• Assess patient, family, and environment for safety risks and communicate changes to the physician and team.
• Develops care plans with measurable goals and coordinated discharge planning.
• Delivers care with evidence-based practices and maintains documentation in the EMR.
