Responsibilities

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Collaborate with the interdisciplinary team to develop, implement, and evaluate care plans.
• Provide skilled nursing interventions including wound care, medication management, IV therapy, and education.
• Deliver emotional support and education to patients and families.
• Supervise LPNs/LVNs and home health aides while coordinating care activities.
• Respond to urgent needs, participate in admissions/on-call, and adapt plans based on monitoring.
• Ensure accurate, compliant clinical records and high-quality care.
