Job summary

Live Well Homecare Agency, LLC is seeking an RN Clinical Nurse to join an interdisciplinary team providing high-quality, patient-centered care in a clinical environment guided by evidence-based standards and policies.

You will conduct assessments, develop and evaluate care plans, and collaborate with patients and families to ensure safety, cultural sensitivity, and effective documentation. The role emphasizes teamwork, ongoing competency, and resourceful care planning.

Qualifications

• Active RN license with BLS certification.
• Bachelor’s or Associate degree in Nursing preferred.
• Strong interpersonal, communication, and critical thinking skills.
• Basic computer proficiency.
• Prior direct patient care experience preferred.

Responsibilities

• Conduct assessments, develop and evaluate care plans, and implement individualized nursing interventions.
• Partner with patients and families, ensuring culturally sensitive care, and promote safety and quality through documentation and collaboration.
• Facilitate communication across disciplines, support performance improvement initiatives, and participate in shared governance to enhance care outcomes.
• Plan patient care proactively, develop protocols, and support efficient use of resources to optimize outcomes.
