Job summary

LiveWell Homecare Agency, LLC is seeking an RN Clinical Nurse to join an interdisciplinary team in a clinical setting. You will provide high-quality, patient-centered care under Nurse Managers, following evidence-based standards and organizational models that promote professional growth.

Responsibilities include conducting the Nursing Process, promoting culturally competent care, mentoring peers, and contributing to performance improvement and policy adherence.

Qualifications

• Requires RN with an Associate’s Degree; BSN preferred.
• Minimum 2 years of direct patient care experience.
• Active RN license (state or multi-state compact), BLS, BLS Instructor, and BLS Provisional.
• Proficiency in clinical technology and basic computer skills.
• Strong communication and teamwork abilities.

Responsibilities

• Perform the Nursing Process—assessment, diagnosis, planning, implementation, and evaluation—using critical thinking and evidence-based practices.
• Deliver relationship-based, culturally competent, patient-centered care aligned with patient and family goals.
• Partner with patients and significant others through effective communication, education, and updating to ensure optimal outcomes.
• Coordinate and document safe, quality care, promoting teamwork, inclusivity, and respectful clinical environments.
• Participate in performance improvement initiatives, research, and shared governance to enhance care quality.
• Support professional development by completing competency assessments, mentoring, and serving as preceptor.
• Advance evidence-based practice, research, and policy adherence, including data collection and protocol development.
• Optimize resource utilization and contribute to care planning across the continuum.
• Perform other duties as assigned and ensure compliance with policies and standards.
