Job summary

Jefferson Health's Albert Einstein Medical Center is seeking a dedicated registered nurse to deliver patient- and family-centered care using the nursing process. You will create and implement individualized care plans in partnership with patients and caregivers, while upholding cultural beliefs and values.

You will collaborate with the healthcare team, engage in education of patients and families, and participate in quality improvement activities to ensure safe, compassionate care across shifts

Qualifications

• Specialized Diploma from an accredited school of nursing.
• Patient- and Family-Centered Practice knowledge.
• Cultural awareness and ability to engage diverse patients.
• Effective communication and delegation within the care team.
• Patient assessment and care planning knowledge.
• Education of patients and caregivers about the condition, treatment and discharge.
• Medication administration and prescribing knowledge.
• Discharge planning and follow-up care coordination.
• Knowledge of nursing assessment/diagnosis and clinical judgement.
• Health information documentation.
• BLS – American Heart Association.

Responsibilities

• Interacts with coworkers, patients and family to deliver culturally respectful care.
• Collaborates with patients and families to identify learning needs and provide understandable education.
• Uses information technology to communicate and monitor care outcomes.
• Works within nursing and interprofessional teams to ensure safe, coordinated care.
