The role emphasizes assessment, planning, implementation and evaluation within nursing standards, with patient- and family-centered education on illness management and safe medication administration.

The nurse will collaborate with the healthcare team to tailor care plans, educate caregivers, and document outcomes while maintaining a commitment to safety and quality in a homecare environment.

Qualifications

• Current RN license is required.
• Nursing diploma or associate’s degree required; bachelor’s degree preferred.
• No prior experience required; two years of experience preferred for advanced roles.
• Strong critical thinking, communication and collaboration skills.

Responsibilities

• Promote healing by educating patients and families on post-treatment care and medication use.
• Participate in activities to ensure patient safety, quality, and regulatory compliance and pursue professional development.
• Develop, implement and document individualized care plans with clear goals in collaboration with the care team and family.
• Assess physical, psychosocial and emotional needs, considering cultural, spiritual and population-specific factors.
• Work in a safety‑sensitive environment with constant alertness and safe practices.
