Job summary

Rock County is seeking a Registered Nurse (RN) for day shift coverage with a 0.6 FTE load and a 6-day week spanning Tue-Thu-Sat-Sun-Thu-Fri. The role includes supervision of licensed practical nurses and nursing assistants, medication administration, IV therapy, and comprehensive care coordination within a skilled nursing setting on the Rock Haven Campus.

A new hire bonus of $2,000 is distributed over the first year.

Qualifications

  • Registered Nurse with current Wisconsin Licensure in good standing.
  • CPR certification or evidence of continued CPR updates.
  • Long-term care experience preferred.

Responsibilities

0.6 RN Day Shift, TU-TH-SA-SU-TH-F

New Hire Bonus of $2,000 distributed over the course of 1st year.

A registered nurse (RN) is a professional nurse who is responsible for leadership, direction and clinical supervision of the health care team including the licensed practical nurses, medication assistants and certified nursing assistants. Accountable for both the provision of nursing care and the coordination of care for the resident in collaboration with the interdisciplinary team. The RN has supervisory responsibility for the licensed practical nurses and nursing assistants in the area of direct patient care.

Administers medications and performs treatments.

1. Administers medications prescribed for routine administration including those given via oral, subQ, IM, nasal, eye, ear, vaginal, rectal, inhaler and nebulizer routes. Observes the effects of medications, reports to the RN Supervisor, Nurse Manager, DON, ADON or MD any unusual or adverse reactions. Assures compliance with the State and Federal guidelines related to medication pass.
2. Administers IV therapy and IV prepared piggyback medications and installs heparin and saline flushes. Completes blood draws.
3. Administers PRN medications as indicated.
4. Performs treatments including: clean and sterile dressing changes, tracheostomy care and changes, skin tear protocol, catheter care and changes, colostomy care, blood sugar checks, G or J tube care and feeding, peritoneal dialysis, oxygen administration, suctioning and nebulizer use.
5. Administers emergency care including CPR.
6. Performs EKGs and bladder scans.

20% - Collects and records data which will be incorporated into nursing assessments and participates in the overall planning and implementation of the resident's plan of care.

1. Reports changes of condition to the nursing supervisor, nurse manager and the physician. Assures continuity of care through the change of shift report.
2. Counsels and provides health teaching to the resident and/or family members based on need.
3. Interacts with residents, families and representatives of other agencies on an ongoing basis.
4. Participates in the overall planning and implementation of the comprehensive and restorative plan of care.
5. Documents medication administration, treatments, treatment progress notes, monthly summaries, comprehensive summaries, quarterly assessments and restorative nursing notes.
6. Reviews electronic health records and co-signs as necessary incident reports, orders and LPN information gathered.

25% - Function as the team leader on the unit as well as during code situations (Code Blue and Red Alert).

1. Assures safe environment for residents, staff, families and visitors.
2. Provides guidance and supervision to the licensed practical nurses, medication assistants and certified nursing assistants.
3. Participates in Quality Assurance and/or other committees as assigned.
4. Attends and required in-service programs in person and online.

10% - Monitors and assists with resident dining.

1. Assures that the care needs of high-risk residents are followed.

5% - Perform other duties as assigned.

  • Knowledge of current theory base in medical, geriatric and psychiatric nursing.
  • Knowledge of current theory in medication administration including psychoactive medications.
  • Knowledge of current theory related to the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs) and Utilization Guidelines. Background in the State and Federal nursing home survey processes.
  • Knowledge of needs of client populations and treatment sources.
  • Ability to communicate effectively orally and in writing.
  • Ability to effectively present information and respond to questions from managers, employees, residents, families, professionals and the general public
  • Ability to work well as a team member. Ability to establish and maintain effective working relationships with physicians, staff and management.
  • Ability to recognize and respond appropriately to urgent/emergent situations.
  • Ability to multi-task efficiently and effectively.
  • Ability to review and assure that the clinical work of others is in accordance with professional standards and practice guidelines.
  • Demonstrates responsibility and accountability for professional practice and for continued professional growth and self-evaluation.
  • Ability to maintain professional appearance and conduct at all times.

  • Performs related work as assigned.
  • Registered Nurse with current Wisconsin Licensure in good standing.
  • CPR Certification or evidence of continued attendance at CPR updates
  • Long-term care experience preferred.
  • May need to stay past the normal shift until relieved in order to meet minimal staffing requirements.
  • Ability to sit, stand, bend, and stoop for periods of time.
  • Ability to lift up to 50 pounds occasionally.
  • Ability to work in a stressful environment.
  • Ability to respond to emergency/crisis situations.
  • Ability to push/pull a medication cart.
  • Exposure to noise.
  • Exposure to blood and/or body fluids.
  • Uses personal protective equipment as needed but not limited to: gloves, gowns, goggles and protective shield.
  • Ability to enter accurate information into charts and reports.
  • May be subject to potentially hostile and emotionally upset residents, family members, personnel, visitors,etc.
  • May be subject to falls, burns, odors, etc., throughout the work day.
  • May be exposed to infectious waste, diseases, blood, body fluids, conditions, etc.
  • Work is inside in a skilled nursing home facility.
  • CPR/AED certification required
