Job description

Willow Tree Care Center - Delta, Colorado

Registered Nurse - RN MDS Coordinator Skilled Nursing Community

One Day Per Week & PRN as needed

Pay Rate - $37.45/hr - $47.08/hr

FUNCTION: Overall coordination of the facility's Medicare Program, Resident Assessment Instrument (MDS), and care planning processes.

QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum Qualifications:

  • A licensed registered nurse with healthcare experience in an acute or long-term care setting preferred.
  • The MDS/Medicare Coordinator activities may be carried out by an LPN/LVN under the supervision of an RN.
  • Current unrestricted state license to practice as a nurse.
  • Current CPR certification; additional certification in nursing specialty preferred.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each key function satisfactorily.

  1. Maintain current knowledge of applicable managed care, Medicare, and state Medicaid regulations.
  2. Maintain current knowledge of federal/state regulations regarding the MDS and care planning process.
  3. Accurately complete all resident assessments and appropriate care plans to meet resident care needs.
  4. Assist with pre-admission screenings to determine and prepare for resident care needs.
  5. Maintain current MDS status of every resident according to state/federal guidelines.
  6. Schedule residents for appropriate admission, quarterly, annual, or change of condition assessments, and oversee communication with nursing and other departments.
  7. Respond to resident condition changes by coordinating re-assessments and revisions to the care plan.
  8. Monitor MDS and care plan documentation on all residents, ensuring complete, organized, and chronological records in the resident chart.
  9. Coordinate and facilitate interdisciplinary team meetings to gather information, communicate changes, and maintain/update records.
  10. Maintain accurate entry of resident information into the computerized MDS program.
  11. Assist in orientation and in-service education programs for staff on MDS, care planning, and Medicare processes.
  12. Ensure that the quality and appropriateness of resident care meet or exceed facility guidelines and comply with state and federal regulations.
  13. Coordinate the implementation of Medicare/PPS program guidelines.
  14. Monitor documentation for compliance with program guidelines, including physician orders, certifications, daily notes, and denial documentation.
