Job summary

AdventHealth in La Grange, IL is seeking a Registered Nurse to provide patient-centered care in a part-time, day-shift role. You will implement care plans, monitor patient status, and coordinate with the healthcare team to ensure high-quality outcomes.

Job Description:

• Serves as the primary coordinator of all disciplines for well-coordinated patient care.

• Notes and carries out physician and nursing orders accurately and promptly.

• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

• Leads and mentors staff to ensure optimal performance and competency in care delivery.

• Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.

• Documents all phases of patient status while responsible for their care.

• Participates in the transfer of patient information to ensure continued quality care.

Education:

• Associate's of Nursing [Required]

• Bachelor's of Nursing [Preferred]

Licences and Certifications:
