Job summary

Jobtailor is seeking a Registered Nurse in Arizona to assess patient needs and plan discharge planning within acute care and Home Health settings. The role involves coordinating care, documenting, and delivering evidence-based nursing while educating patients and families.

Candidates should hold an active RN license, BLS, and meet competency requirements with experience in acute care within the past year. Arizona Fingerprint clearance and valid driver’s license may apply depending on assignment.

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification required.
• Additional certification or continuing education may be required based on area of practice.
• Experience in the clinical area for which applying is desired.
• Must maintain clinical performance competencies via annual validations.
• Banner Registry and Travel positions require minimum of one year in an acute care hospital setting.
• Experience must include working in an acute care setting within the past 12 months as RN in the specialty area.
• Banner Registry and Travel physician practice positions require minimum of one year RN experience in a physician practice or acute care setting.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Reviews patient history and physical with patient/family and ensures completion within timeframe.
• Formulates a plan of care, including the discharge plan, using assessment data and input from patient/family and health team.
• Implements the plan of care through direct patient care, coordination, delegation and supervision of health care team activities.
• Evaluates the patient’s and family’s response to care and teaching, and revises the plan of care as needed.
• Documents assessment, planning, implementation and evaluation in the patient record.
• Provides care based on the best evidence available and may participate in research activities within clinical practice.
• Interacts and participates in education, role development, and orientation of facility personnel, patients, students, families and visitors.

• Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting
• Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area
• Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
• In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license.
• Auto insurance coverage is also required to be active and maintained.
• Employees working at Banner Behavioral Health Hospital, BTMC Behavioral or BUMG, BUMCT in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
• An Arizona Criminal History Affidavit must be signed upon hire.
