The successful candidate:
- Provides disease management with knowledge of oncology, chemotherapy, and side effects; monitors and addresses symptoms. Educates patients and families about their disease, treatment plans, and quality of life issues. Documents assessments and interventions promptly in the electronic medical record. Collaborates with and supports the provider team in managing and coordinating patient care. Evaluates health issues presented by patients and caregivers, communicating pertinent issues effectively.
- Triages patient medical issues efficiently, either in person or by phone. Intervenes appropriately through orders or consultations with the healthcare team. Provides professional nursing services in cooperation with clinical and clerical team members. Collaborates with team members to develop multidisciplinary care plans and sets patient-centered goals.
- Acts as a resource within the healthcare team to facilitate communication and services among physicians, patients, families, and external agencies. Provides patient education using appropriate resources and documents this in the EHR.
- Performs tasks according to facility policies, procedures, regulations, and nursing best practices, including but not limited to central line care, patient education, procedure assistance, and medication administration. Assists with medication access, precertification, and ensures proper utilization of services to maximize reimbursement and patient satisfaction.
Additional Responsibilities:
- Works with the healthcare team to deliver excellent patient care, increasing satisfaction and retention. Aims for positive outcomes and resolves variances. Supports team members to ensure efficient service delivery. Develops collaborative relationships with other departments and external health agencies to facilitate care.
- Must be able to work onsite from 8:00 am to 4:30 pm, Monday through Friday.
Qualifications
Your qualifications should include:
- 3-5 years of clinical nursing experience
- Current Illinois RN license
- BSN or higher preferred
- Basic Life Support (BLS) certification through the American Heart Association
- Oncology Certified Nurse (OCN) certification preferred
- Bio/Chemotherapy certification preferred
