Huntsville Post-Acute and Rehabilitation Center in Huntsville, TN is seeking a Registered Nurse (RN) to coordinate and deliver resident care in our skilled nursing environment. You will lead an interdisciplinary team to ensure high-quality, person-centered services and compliant documentation.

The role emphasizes clinical assessment, delegation, and collaboration with physicians, therapists, and social services to support discharge planning and resident well-being.

Qualifications

• Must hold an associate or higher nursing degree.
• Current, unencumbered RN license in Tennessee.
• Experience in long-term care/skilled nursing preferred.
• Strong clinical assessment and communication skills required.
• Ability to manage multiple priorities in a fast-paced setting.

Responsibilities

• Complete comprehensive and focused assessments and develop individualized care plans.
• Provide and oversee direct nursing care, including treatments and medication administration.
• Lead and support LPNs, CNAs, and CMAs with safe delegation.
• Coordinate resident care and discharge planning with physicians and therapies.
• Communicate with residents and families about care plans and changes in condition.
• Ensure accurate documentation and participate in quality improvement and regulatory activities.
• Promote safety, teamwork, and resident-centered care.

Position Summary

The Registered Nurse (RN) is responsible for overall coordination and delivery of resident care, leading the interdisciplinary team to ensure high-quality, person-centered services in our skilled nursing environment.

Responsibilities

• Complete comprehensive and focused assessments and develop, implement, and update individualized care plans.
• Provide and oversee direct nursing care, including treatments, medication administration, and clinical interventions.

Qualifications

• Current, unencumbered RN license in the state of Tennessee.
• Long-term Care/skilled Nursing Experience Preferred.
