Position Purpose
The Saint Francis Hospital Outpatient Heart Failure Program provides care to our patients from our surrounding communities experiencing every stage and type of heart failure. The program has achieved Gold Plus status in Heart Failure from the American Heart Association and Advanced Certification in Heart Failure from The Joint Commission, recognizing high standards in treatment. The outpatient clinic specializes in various degrees of heart failure, focusing on improving patients’ quality of life and disease management through guideline-directed medical therapy (GDMT) and evidence-based care. Clinic leadership will support your orientation and tailor the process to meet individual needs.

The Nurse Navigator plays a vital role within our program. Collaborating with the inpatient team, you will identify heart failure patients and equip them with tools for successful disease management post-discharge. You will provide initial bedside education, connect patients to outpatient services, and work closely with MDs and APPs on managing medical issues and facilitating medication and care resources. You may also provide infusion therapy for diuresis and coordinate outpatient appointments for medication guidance and disease management. This role involves teamwork within a multidisciplinary team including MDs, APPs, RNs, and MAs, all committed to quality patient care.
