Job description

The Heart and Vascular Center at Dartmouth-Hitchcock is seeking a motivated Advanced Practice Provider (APRN or PA) to join our Advanced Heart Failure team. The candidate should have a strong interest and background in general cardiology and/or advanced heart failure. The candidate would be joining a diverse section comprised of both physicians and APPs, with the opportunity to collaborate with innovative cardiology subspecialties such as electrophysiology, pulmonary hypertension, interventional and structural heart disease, and cardiac surgery. The heart failure program also manages cardiac transplant and left ventricular assist device (LVAD) patients, as part of a shared-care model with Tufts Medical Center in Boston.

The APP would be dedicated to both inpatient and ambulatory clinical settings, helping to provide and coordinate high quality care and education to heart failure patients and their families as part of a multidisciplinary team comprised of physicians, APPs, and nurses. This position allows a variety of responsibilities, including the assessment, diagnosis, and management of complex and acute cardiac conditions, new presentations of cardiomyopathy and heart failure, cardiogenic shock, temporary mechanical circulatory support, and evaluation for transplant and LVAD candidacy. The position also involves management of chronic heart failure, including integration of palliative care, when appropriate. The individual selected will act as a patient advocate, assuring that all aspects of the patients' and families' needs are met and fragmentation of care is prevented.

In addition to clinical responsibilities, the scope of practice includes a teaching role and participation in quality improvement activities. As the home to the Dartmouth Institute and Geisel School of Medicine, opportunities for academic appointment and advancement are rich for motivated providers.

This position hosts hours primarily between ~7:30a-5:30p on weekdays. Options for 4 or 5 day workweeks are available. There are no weekend, holiday or call responsibilities.
