Job summary

Jefferson City Medical Group seeks an Oncology Nurse Navigator (RN) to coordinate care for oncology patients, educate patients, and advocate for timely, seamless treatment. The role requires skilled communication, care coordination, and proficiency with EHR systems.

You will work with the multidisciplinary team to develop comprehensive care plans, manage multiple processes, and support patients and families through treatment and survivorship planning.

Qualifications

  • 2+ years of oncology nursing experience
  • MO RN licensure (state)
  • Oncology nurse certification preferred or obtainable within 12 months

Responsibilities

  • Assist with new patient intake and schedule appointments.
  • Coordinate staging/restaging workups and ensure chart accuracy.
  • Track referrals and confirm appointments across providers.
  • Monitor hospitalized patients and arrange follow-up care.
  • Serve as primary contact for patients and connect to resources.
  • Educate patients on diagnosis and treatment; assist with chemo/infusion education.
  • Participate in tumor board and document navigation activity in the EHR.
  • Provide triage and route to appropriate care level.
  • Assist with required documentation for assistance programs and funding.
  • Organize care plans and communicate with patient/family and multidisciplinary team.

Job description

Responsibilities

The Oncology Nurse Navigator is a Registered Nurse with experience in Oncology who acts as a care coordinator, educator, advocate, counselor to achieve optimal outcomes for patients. The Oncology Nurse Navigator addresses patient needs through the continuum to reduce gaps in the care process and provide education, improve timeliness of care and coordinate complex care process, and develop plans for long-term and survivorship care.

Job specific competencies
  • Assist with new patient intake and schedule clinic, specialist and treatment appointments.
  • Coordinate staging/restaging workups (imaging, labs, pathology) per physician orders; keep chart summary current and flag key findings to the oncologist.
  • Track referrals to specialists, surgeons, and radiation oncology; confirm appointments are kept, records are returned, and patients follow back to the clinic.
  • Monitor patients hospitalized for any reason; ensure a follow-up appointment is scheduled and gather discharge records for the chart.
  • Serve as primary point of contact for patients; address barriers to care and connect patients to resources.
  • Educate patients on diagnosis and treatment; assist providers with chemo/infusion education as needed.
  • Participate in tumor board, document navigation activity in the EHR.
  • Provide triage/symptom management and route the patient to appropriate level of care.
  • Inform and assist patients and/or family members when completing required documentation for assistance programs, copays, medication cards, charity foundations, etc.
  • Organize care plans and communicate same to patient/family and collaborates with multidisciplinary team members.
  • Ability to manage multiple processes and work effectively with other team members.
  • Assist with other nursing duties as per clinic needs.

Minimum Qualifications
Education
  • Graduation from an accredited school of nursing, BSN preferred
Experience
  • At least 2 years of experience as an oncology nurse
Certification/License
  • Licensure as a registered nurse in the state of MO
  • Oncology nurse certification preferred/ or to be obtained within 1 year of employment
Knowledge/Skills/Abilities
  • Ability to comprehensively assess patient’s physical and psychosocial status
  • Ability to communicate openly and empathetically with patients, families and physicians
  • Working knowledge of computer applications such as Microsoft Office and patient management database products
  • Superior written and verbal skills are essential
WORK ENVIRONMENT

Works in heated and air-conditioned area; may be exposed to infectious material and contagious conditions; frequently exposed to patient body fluids, including blood and contaminated sharps; potential for exposure to hazardous/disinfecting chemicals and radiation.
