Job summary

JCMG is seeking a Registered Nurse Navigator - Oncology in Jefferson City, MO. This on-site role coordinates patient intake, treatment scheduling, and interdepartmental communication to ensure timely, coordinated cancer care.

The RN navigator educates patients on diagnoses and treatments, supports the care team, and participates in tumor board activities to optimize outcomes and survivorship planning.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of 2 years in oncology nursing.
• MO RN licensure; oncology certification preferred or to be obtained within 1 year of employment.

Responsibilities

• Coordinate new patient intake and schedule clinic, specialist, and treatment appointments.
• Coordinate staging/restaging workups and keep chart summaries current.
• Track referrals to specialists and ensure appointments and records are managed.
• Monitor hospitalized patients and schedule follow-up visits.
• Serve as primary patient contact, addressing barriers to care.
• Educate patients on diagnosis and treatment and assist with chemo education.
• Participate in tumor board and document navigation in the EHR.
• Provide triage and route to appropriate care level.

RESPONSIBILITIES

The Oncology Nurse Navigator is a Registered Nurse with experience in Oncology who acts as a care coordinator, educator, advocate, counselor to achieve optimal outcomes for patients. The Oncology Nurse Navigator addresses patient needs through the continuum to reduce gaps in the care process and provide education, improve timeliness of care and coordinate complex care process, and develop plans for long‑term and survivorship care.

Job specific competencies:

• Assist with new patient intake and schedule clinic, specialist and treatment appointments.
• Coordinate staging/restaging workups (imaging, labs, pathology) per physician orders; keep chart summary current and flag key findings to the oncologist.
• Track referrals to specialists, surgeons, and radiation oncology; confirm appointments are kept, records are returned, and patients follow back to the clinic.
• Monitor patients hospitalized for any reason; ensure a follow-up appointment is scheduled and gather discharge records for the chart.
• Serve as primary point of contact for patients; address barriers to care and connect patients to resources.
• Educate patients on diagnosis and treatment; assist providers with chemo/infusion education as needed.
• Participate in tumor board, document navigation activity in the EHR.
• Provide triage/symptom management and route the patient to appropriate level of care.
• Inform and assist patients and/or family members when completing required documentation for assistance programs, copays, medication cards, charity foundations, etc.
• Organize care plans and communicate same to patient/family and collaborates with multidisciplinary team members.
• Ability to manage multiple processes and work effectively with other team members.
• Assist with other nursing duties as per clinic needs.

MINIMUM QUALIFICATIONS

Education

• Graduation from an accredited school of nursing, BSN preferred

Experience

• At least 2 years of experience as an oncology nurse

Certification/License

• Licensure as a registered nurse in the state of MO
• Oncology nurse certification preferred/ or to be obtained within 1 year of employment

Knowledge/Skills/Abilities

• Ability to comprehensively assess patient’s physical and psychosocial status
• Ability to communicate openly and empathetically with patients, families and physicians
• Working knowledge of computer applications such as Microsoft Office and patient management database products
• Superior written and verbal skills are essential

WORK ENVIRONMENT

Works in heated and air-conditioned area; may be exposed to infectious material and contagious conditions; frequently exposed to patient body fluids, including blood and contaminated sharps; potential for exposure to hazardous/disinfecting chemicals and radiation.
