Job summary

Concordia Visiting Nurses is seeking a Home Health Registered Nurse (RN) to provide personalized, one-on-one patient care in home settings within our service area. You will assess, plan, and implement care in collaboration with physicians, patients, and families.

Responsibilities include skilled treatments (IV starts, wound care, chest tube management when applicable), patient education, and ongoing condition monitoring to promote safe self-management and independence.

Qualifications

• Minimum one year of recent clinical experience as an RN recommended.
• Home health experience recommended.
• Adaptability, compassion, and patient-centered care.

What you’ll do as a Registered Nurse RN:

As a Home Health RN, you'll be at the heart of this mission.

Conduct thorough assessments and developindividualized care plansin collaboration with physicians, patients, and families.

• Treatments, IV starts, Chest Tube Management, and Wound care.

• Educating and empowering patients and families on managing their health conditions and building confidence in self-care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
