Job summary

Concordia Visiting Nurses, part of Concordia Lutheran Ministries, seeks a Home Health Registered Nurse (RN) to deliver compassionate, one-on-one patient care in home settings. You will assess patients, develop care plans with physicians, and manage treatments such as IV starts, wound care, and other clinical needs.

Educate patients and families, monitor conditions, and adjust plans as needed, while enjoying autonomy, flexible scheduling, and a collaborative, mission-driven team.

Qualifications

• Minimum one year of recent clinical experience as an RN.
• Home care experience recommended.
• Adaptability, compassion, and patient-centered care.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.
• Treatments, IV starts, Chest Tube Management, and Wound care.
• Educating and empowering patients and families on managing their health conditions and building confidence in self-care.
• Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
