A 99-bed facility, Raker Center is open to residents ages 18 and older with primary diagnoses including, but not limited to, cerebral palsy, muscular dystrophy, quadriplegia or multiple sclerosis. An interdisciplinary team seeks to help residents maintain as much independence as possible for the longest amount of time. The team consists of attending physicians, neurologists, psychiatrists and psychologists, RNs and LPNs, social workers, chaplains, and physical, speech, occupational and recreational therapists.

Job Summary

Promotes and restores residents' health by completing the nursing process. Collaborating with physicians and interdisciplinary team members. Provides physical and psychological support to residents and families. Delegates and evaluates care rendered by LPNs and CNAs to meet resident care needs.

Essential Functions
  • Identify resident care requirements
    • Utilize the Nursing Process as a framework to deliver care by establishing or contributing to a plan of care with the resident
    • Establish and contribute to prioritization of goals with the resident and interdisciplinary team
    • Identify nursing diagnoses
    • Identify teaching objectives appropriate to care standards
    • Evaluate effectiveness of nursing interventions and resident progress toward goals as resident condition changes
    • Continuously identify actual and potential problems after implementation of nursing interventions
    • Communicate plan of care and residents’ progress to interdisciplinary team
    • Delegate plan of care to other licensed and unlicensed associates within their scope of practice
    • Establish rapport with potential and actual residents and other persons in a position to understand care requirements
    • Provide holistic care that addresses physical, emotional, cognitive, social, and spiritual needs
  • Establish a compassionate environment
    • Provide emotional, psychological, and spiritual support to residents, families, caregivers, and interdisciplinary team members
  • Promote resident independence
    • Establish care goals with residents
    • Educate residents, family members, and friends to understand condition, medications, and self‑care skills
    • Demonstrate competence in providing care to residents
  • Assure quality of care
    • Adhere to therapeutic standards
    • Measure health outcomes against resident goals and standards
    • Continually recommend appropriate adjustments to resident plan of care
    • Comply with organizational and nursing division’s philosophies and standards of care set by state boards and other governing agencies
    • Participate in performance improvement activities by directing and assessing care provided to residents by RNs, LPNs, CNAs, agency personnel, students, and other associates
  • Resolve resident problems & needs
    • Utilize interdisciplinary team strategies
    • Utilize established communication methods to ensure resolution of resident problems and needs
    • Communicate resident status to interdisciplinary team members
  • Document resident care services
    • Document delivery of service, changes in resident condition, and resident response to nursing interventions in the resident medical record and other departmental records according to policies and procedures
    • Ensure documentation of care rendered by other staff members
    • Review, transcribe, communicate, and implement physician orders
  • Maintain confidentiality
    • Comply with HIPAA
    • Ensure that all resident and organizational information is kept confidential
  • Maintain safe & clean environment
    • Comply with Infection Control policies, procedures, and regulations
    • Comply with policies and procedures on storage and disposal of controlled substances, toxic and non‑toxic chemicals, and other wastes
    • Coordinate with health care support personnel
  • Ensure resident safety
    • Demonstrate skills and knowledge in compliance with the policies and procedures on medication and treatment administration
    • Ensure CNAs and other staff members’ compliance on resident appropriate transfers, feeding, and personal hygiene
    • Respond to resident medical and psychological concerns, potential or actual injury and distress
    • Comply with the policies and procedures on handling resident abuse
  • Ensure operation of equipment
    • Demonstrate knowledge and skills on appropriate usage of equipment according to manufacturer’s instructions necessary in the delivery of care to residents
    • Include troubleshooting malfunctions and requesting maintenance orders for repairs
    • Participate in evaluation and recommendation of new equipment
  • Demonstrate financial accountability
    • Ensure availability and inventory of stock supplies
    • Utilize equipment and/or supplies as needed to accomplish job results
    • Demonstrate time management skills through prioritization and delegation of tasks
    • Comply with nurses’ roles on reimbursement for resident care services
  • Maintain professional & technical knowledge
    • Identify professional education needs and opportunities
    • Attend educational workshops
    • Complete mandatory educational requirements

  • Education
