Qualifications

Responsibilities

• Develop and evaluate plan of care for each patient.
• Assess knowledge, physical, psychosocial, and spiritual health via interviews and exams.
• Collaborate with multidisciplinary team on nursing assessments.
• Document care and plan of care on charts and records.
• Provide patient education and discharge planning.
• Participate in quality improvement activities and unit meetings.
