Responsibilities

• Monitors, examines, and evaluates patient conditions and coordinates care plans.
• Delivers safe and effective care according to hospital policies and procedures.
• Communicates care plans to patient, family, and care team; prioritizes patient flow and experience.
• Participates in performance improvement initiatives and professional development.
• Maintains documentation of assessments, care delivered, and any incidents.
