Responsibilities

The Registered Nurse (RN) in Home Health provides and manages nursing care to patients in their homes, following physician prescriptions and legal regulations. The RN coordinates care plans with healthcare professionals and ensures continuity of care through effective communication with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.

• Provide clinical services within the scope of practice as defined by state laws, according to the care plan, and in coordination with the healthcare team.
• Conduct initial and comprehensive nursing evaluations, develop care plans within state guidelines, and document accurately and timely.
• Regularly evaluate and re-evaluate patient needs, at least every 60 days or as conditions change.
• Perform comprehensive assessments and develop appropriate plans of care in collaboration with physicians.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Develop, implement, and revise care plans in collaboration with healthcare professionals.

Education and Experience

License Requirements:

• Current RN licensure in Louisiana
• Current CPR certification
• Valid Driver’s License, vehicle insurance, and access to reliable transportation

Additional State Requirements:

• Minimum of one year of clinical nursing experience; experience as an LPN may be considered if recent clinical experience is demonstrated. RN licensure must be unrestricted.
