Responsibilities

• Establish personalized relationships with patients and support systems.
• Collaborate with healthcare team to assess, plan, implement, and evaluate care.
• Utilize evidence-based practices and compassion in patient navigation.
• Provide and document patient care under supervision.

Qualifications

• Graduate of an accredited School of Nursing; Bachelor's preferred.
• 1 year of relevant experience preferred.
• Current RN licensure in applicable state.
• Additional skills: basic nursing skills, multitasking, medication administration, communication, computer skills, ability to care for diverse patient populations.

Physical Demands

Constant standing, walking, bending; lift/push/pull 100 lbs.; use of safety equipment; travel between facilities.

Other Requirements

• Use of safety equipment such as HEPA masks for infection control.
• On-call responsibilities.
• Ability to travel as needed.
