Job Summary and Qualifications

Responsible for providing direct patient care in accordance with physician orders, recognizing standards of practice, and establishing company policies and procedures. Responsible for following the plan of care, providing treatments, and working collaboratively with the team to achieve positive patient outcomes.

What you will do in this role:

  • Follow the plan of care, provide treatments, and collaborate with team members to ensure positive patient outcomes.
  • Provide direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Participate in coordinating hospice services, reporting identified needs to the interdisciplinary team, including Hospice Aide, Director of Clinical Services – Hospice, and Chaplain.
  • Manage pain and symptoms appropriately.
What qualifications you will need:
  • Basic Cardiac Life Support (BLS) certification obtained within 30 days of employment start date.
  • Registered Nurse license.
  • Bachelor's Degree in Nursing.
