Job summary

Mental Health Association of Rochester is seeking a Home Infusion Nurse to deliver expert infusion therapy in patients' homes and clinical settings, coordinate referrals, educate patients and caregivers, and develop personalized care plans.

You will work with interdisciplinary teams, provide vascular access procedures, and participate in on-call rotations, with a strong emphasis on safety, quality, and compassionate, patient-centered care.

Qualifications

• Three years of RN experience; home infusion and care experience preferred.
• Graduation from a Specialty Training Program in Nursing is required.
• Strong communication, clinical assessment, and interpersonal skills are essential.

Role Overview:

The Home Infusion Nurse provides expert infusion therapy, patient education, and case management in home and clinical settings. This position involves coordinating referrals, developing personalized care plans, administering infusion medications, and performing vascular access procedures. The nurse collaborates with interdisciplinary teams to ensure safe, effective, and patient-centered care throughout the treatment process.

Key Responsibilities:

• Manage intake and referrals, develop care plans, administer infusions, educate patients/caregivers, monitor therapy, coordinate with healthcare providers, maintain documentation, respond to concerns, and participate in quality initiatives. Supports 24/7 coverage through on-call rotations and travels to patient homes and clinics.
• Supervision & Collaboration: Provide clinical oversight to Licensed Practical Nurses, ensuring adherence to standards and supporting professional growth.

Qualifications & Skills:

Minimum of three years RN experience; home infusion and care experience preferred. Requires graduation from a Specialty Training Program in Nursing. Strong communication, clinical assessment, and interpersonal skills essential.
