Job Description

Functions independently and is accountable for utilizing the nursing process. Coordinates patient care with the Multidisciplinary Team, delegates tasks appropriately, and identifies hospital resources. Serves as a clinical resource for new employees and demonstrates positive leadership within the unit or department.

In Virtual Setting:

Functions independently, demonstrating increasing judgment in data interpretation and prioritization. Initiates actions to improve nursing practice, patient care, and unit processes. Acts as a resource for colleagues, delegates tasks, assumes preceptor responsibilities, and utilizes hospital resources. Provides tele-nursing care to a diverse patient population across various departments, focusing on patient advocacy, prevention, telecommunication, proactive monitoring, evidence-based care, clinical decision support, and compassionate care. Monitors patient data via intelligent software, alerts bedside staff to changes, and partners with them to address concerns, aiming to identify early signs of issues and reduce complications.

  • Monitor intelligent software and vitals for changes or trends.
  • Utilize effective communication skills in tense situations.
  • Leverage experience to interpret trending data for this population.
  • Partner with bedside teams to monitor patients.
  • Detect early signs of potential problems.
  • Notify bedside staff of issues and assist in resource activation.
  • Provide additional monitoring using intelligent software.
  • Identify early signs to reduce complications.

In Community Hospital Setting:

  • Manage care within established nursing standards and state regulations.
  • Prioritize patient needs with a caring, responsive attitude.
  • Initiate stabilization and collaborate with physicians on life support and transfer procedures.
  • Gather detailed patient history and symptoms, including vital signs.
  • Collect laboratory samples and perform tests per regulations and policies.
  • Report adverse treatments or medications as per policies.
  • Prepare patient meals.
  • Participate in QAPI activities, accreditation, and compliance standards.

In Hospital in Your Home Setting:

  • Provide in-home nursing services and patient care.
  • Assess physical and psychosocial patient needs.
  • Collaborate with the healthcare team to ensure optimal outcomes.
  • Execute physician orders and communicate patient needs.
  • Participate in ongoing training and education.
  • Educate and counsel patients and families.
  • Document visits, interventions, and educational activities.
  • Follow legal, system, and policy guidelines.
  • Perform other duties as assigned.

In ASC Setting:

  • Provide professional nursing care following established standards.
  • Handle telephone triage, patient flow, and coordination.
  • Schedule urgent appointments, identify learning needs, and perform procedures like IV insertion.
  • Delegate tasks based on competencies and scope of practice.
  • Serve as a subject matter expert in patient education and assessment.
  • Work effectively in team and autonomous settings.
  • Maintain excellent customer service and professionalism.
  • Assist with office duties as needed.
  • Gather patient history and symptoms, including vital signs.
  • Perform lab testing and report adverse treatments as per policies.
  • Participate in accreditation and quality activities.
Job Requirements

Experience: 2 years of relevant nursing experience. Certifications: Basic Life Support (American Heart Association or Red Cross). Education: Registered Nurse with an Associate Degree or higher.
