This role focuses on monitoring patients’ functional status and coordinating care within a multidisciplinary team. The position requires completion of a nursing program and an active RN license (Oklahoma or multi-state).

Qualifikationen

• Graduate of an approved nursing program and eligible for licensure.
• Current valid RN license (multistate or Oklahoma).
• 0–6 months of related nursing experience.

Aufgaben

• Utilizes the nursing process to care for patients.
• Assesses, organizes, and completes patient care with consideration for age, culture and psychosocial factors.
• Documents steps of the nursing process.
• Addresses problems and progress toward goals.
• Facilitates patient discharge through timely referrals and patient education resources.
• Collaborates with other disciplines and participates in unit education and PI activities.
