Job description

Responsible for planning, organizing, directing, implementing, and evaluating all aspects of resident care to meet residents' needs and ensure compliance with State and Federal laws, as well as community policies and procedures.

Responsibilities

1. Communicate with nursing staff, physicians, pharmacists, family members, and others regarding residents' status.
2. Address concerns from residents, families, or physicians, initiating corrective actions or reporting them to appropriate personnel.
3. Attend Resident Care conferences; maintain accurate and relevant Resident Care Plans.
4. Prepare, administer, and document medications per physician orders; systematically reorder medications and ensure expired medications are replaced.
5. Document patient care issues accurately and report them as required.
6. Recognize signs and symptoms, using judgment to determine appropriate treatments or procedures.
7. Supervise ancillary personnel, providing direction and initiating corrective actions as needed.
8. Perform other duties as assigned.

Qualifications

Basic Qualifications & Experience

1. Advanced training in geriatrics preferred.
2. At least 1 year of experience in a long-term care facility preferred.
3. Supervisory experience preferred.
4. Proficient in Microsoft Office applications (Excel, Word, Outlook).
5. Ability to read, write, and speak English fluently.
6. Excellent customer service skills.

Working Conditions / Physical Requirements

• Moderate physical activity; handling objects up to 50 pounds occasionally and 20 pounds frequently.
• Potential exposure to chemicals, blood-borne pathogens, and infectious diseases.
• Frequent sitting, standing, bending, and moving during work hours.
• Subject to interruptions and frequent interaction with residents, families, and visitors.
