Job summary

Optum is seeking a Registered Nurse for per diem Hospice care (PRN) to evaluate and coordinate care for hospice patients. The role requires weekday and weekend availability, with hours from 8:00 AM to 4:30 PM, and training during weekday hours as needed.

The RN will assess patient and family needs, initiate and monitor the plan of care, document patient data, and collaborate with the care team to ensure high-quality hospice services.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re‑assesses as needed, no less than every fifteen (15) days.
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.

Position is PRN, weekday and weekend availability needed, 8am-4:30pm. Must be willing to train during weekday hours.

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Preferred Qualification:

• Current CPR Certification or ability to complete within 90 days of hire

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $37.40 to $56.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
