VitalCaring is seeking a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered end-of-life care in field-based settings across Baytown, Clearlake, and Webster. You will lead plans of care, manage symptoms, and coordinate with an interdisciplinary team to ensure comfort and dignity for patients and families.

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Role Overview

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments

• Act as case manager, coordinating care across the interdisciplinary team

What You Bring

• Active RN license (or compact eligibility)
