Job summary

VitalCaring is seeking an On Call Hospice RN to lead the plan of care, manage symptoms, and provide comfort-focused care in patient homes and facilities across the service area. This field-based role emphasizes presence, empathy, and clinical excellence, with autonomy and support from an interdisciplinary team.

You’ll coordinate care, educate families, document meticulously, and participate in care planning meetings.

Qualifications

• Active RN license (or compact eligibility) and current CPR/ACLS
• Minimum 1 year of clinical nursing experience
• Proficient in EMR systems and charting
• Ability to manage a caseload independently and communicate with patients and families clearly

Responsibilities

• Lead Patient-Centered Hospice Care: perform assessments, develop and manage comfort-focused plans, deliver skilled nursing care
• Serve as the Care Coordinator: act as case manager, coordinate across interdisciplinary team
• Support Patients & Families: provide education and emotional support, help with complex decisions
• Drive Quality & Compliance: maintain documentation and participate in care-planning meetings

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

What You’ll Do

• Lead Patient-Centered Hospice Care
  • Conduct comprehensive patient and family assessments
  • Develop and manage individualized, comfort-focused care plans
  • Deliver skilled nursing care with a focus on pain and symptom management
• Serve as the Care Coordinator
  • Act as case manager, coordinating care across the interdisciplinary team
  • Collaborate with physicians, social workers, chaplains, and aides
  • Monitor patient condition and adjust care plans to maintain comfort
• Support Patients & Families
  • Provide education, emotional support, and guidance throughout the journey
  • Help families navigate complex decisions with empathy and clarity

• Drive Quality & Compliance
  • Maintain accurate, timely documentation
  • Participate in IDT meetings and care planning

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation
