Job summary

Bluegrass Care Navigators is seeking a HospiceFlexRN to support Hospice RN Case Managers and ensure high-quality care for patients in private residences, nursing facilities, personal care homes, or hospitals. The role emphasizes compassionate, autonomous nursing across diverse home settings.

You will participate in on-call rotations, manage multiple visits, assist with admissions and referrals, and collaborate with families and interdisciplinary teams to maintain comfort and quality of life

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse in Kentucky or a Compact State.
• Minimum of one year of general nursing experience preferred; new graduates considered.
• Demonstrates strong clinical nursing skills and professionalism in emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect.
• Adheres to professional, ethical, and legal nursing standards.
• Maintains CPR certification.
• Must be a licensed driver with a vehicle in good working order and insured.

Responsibilities

• Support Hospice RN Case Managers to ensure patient needs are met and high-quality care is maintained in home and facility settings.
• Provide care in private residences, skilled nursing facilities, personal care homes, or hospitals.
• Involve in caseload management, patient visits, and referrals.
• Participate in on-call rotations to ensure continuous patient care.
• Interact with patients, families, and interdisciplinary team members with empathy and professionalism.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Position Qualifications
