What You'll Do:

• Collaborate with our interdisciplinary team to assess patient and family needs, developing personalized nursing care plans and comprehensive service strategies.
• Provide expert nursing care and support to hospice patients in their homes, ensuring their comfort and well-being.
• Supervise and mentor our dedicated team of Home Health Aides and Licensed Practical Nurses, fostering a collaborative and supportive work environment.
• Serve as a vital resource and point of contact for patients, families, caregivers, and other healthcare professionals.
• Participate in our rotating on-call schedule (approximately every 10 weeks) to provide after-hours support to our hospice patients.
• Contribute to our commitment to quality care through participation in quality assurance programs.
• Maintain clear and open communication with all members of the care team, patients, and their families.
• Stay up-to-date with the latest advancements in hospice and home care practices.
• Embrace opportunities to educate the community about hospice and home care services, helping to grow our reach.
• Uphold the long-standing mission of Our Lady of Peace Hospice & Home Health Care, respecting the dignity and privacy of each individual we serve.

What You'll Bring:

• Current Registered Nurse (RN) licensure in the State of Minnesota.
• Preferred: One year of nursing experience, particularly in hospice and/or home care.
• Graduation from an accredited school of nursing.
• Current Minnesota driver's license and valid car insurance.
• CPR Certification.
• Strong understanding of nursing theory and practice, the nursing process, and current medical and nursing interventions for the terminally ill.
• Knowledge of hospice philosophy and Medicare hospice eligibility and regulations.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• The ability to empathize and a true wish to provide support to patients and families in their end-of-life journey.
• Ability to cope with the emotional demands of hospice care and manage multiple priorities.

• Competitive Compensation: Work 32-40 hrs/week. Earn $45 - $47/hour, Monday through Friday.
