Pallatus Healthcare is seeking a compassionate Hospice Registered Nurse to lead clinical care in homes, facilities, and community settings, ensuring comfort and dignity for patients in their final stages of life.

You will work with families and an interdisciplinary team to deliver comprehensive, patient-centered end-of-life care while adhering to hospice regulations and quality standards.

Qualifications

• Current RN license in the state of practice.
• Minimum 2 years of clinical nursing experience.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and make sound clinical judgments.
• Comfortable discussing end-of-life issues with patients and families.

Responsibilities

• Provide skilled nursing assessments, care planning, and direct patient care for hospice patients.
• Manage pain and symptom control protocols under physician supervision.
• Educate patients and families on disease progression, comfort measures, and what to expect.
• Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains.
• Respond to urgent calls and provide crisis intervention when needed.
• Document patient care activities and maintain accurate medical records.
• Support families through the dying process and provide bereavement guidance.
• Ensure compliance with hospice regulations and quality standards.
• Provide mentorship and guidance to nursing assistants and volunteers.

Position Overview

As a Hospice Registered Nurse at Pallatus Healthcare, you will be the primary clinical coordinator for hospice patients, providing skilled nursing care in homes, facilities, and community settings. You'll work closely with patients, families, and our interdisciplinary team to ensure comprehensive, compassionate end-of-life care that honors each patient's wishes and values.

Preferred Qualifications

• Certification in Hospice and Palliative Nursing (CHPN)
• Experience with pain management and comfort care techniques
• Experience with electronic health records
• Background in home health or community nursing
