Halifax Health ExpressCare is seeking a Registered Nurse for the Hospice After Hours Team in Florida. The RN will coordinate and implement the plan of care to ensure the best life quality for terminally ill patients across hospice settings.

After-hours and weekend coverage, as well as on-call responsibilities, are required. Requires Florida RN license, an Associate degree in nursing (Bachelor’s preferred), and a valid driver's license with a clean driving record.

Qualifications

Responsibilities

• Provide all care aspects for terminally ill patients.
• Educate patients and families.
• Complete initial and ongoing assessments; manage pain and symptoms.
• Develop and implement an Interdisciplinary Plan of Care centered on pt./family goals.
• Use critical thinking in care planning; participate in QA activities.
• Maintain positive professional relationships with care team.

Day (United States of America)Registered Nurse- Hospice (After hours Team)The Hospice RN will be responsible for coordination and implementation of the overall plan of care for the assigned patients in various hospice settings to assure the best possible quality of life is attained.- Currently licensed as an RN in State of Florida. - Associate degree from an accredited School of Nursing required. Bachelor's degree a plus. - Valid driving license and good driving record are required for this position. - Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians and ancillary department personnel. - Personal professional development, accountability, organization and leadership in the performance of professional nursing practice. - All full time RNs will assist in the provision of after hours and weekend coverage as assigned. - On-Call responsibilities required.
