Job summary

Affinity Hospice Management LLC is seeking an on-call Registered Nurse to coordinate and provide direct home and facility-based hospice care in Broward County. You will work nights, be on-call, and support patients and families during critical moments.

You will manage comfort and symptom control, assist with admissions and discharges, document care, and collaborate with the interdisciplinary team. Florida RN license, CPR, and valid driver's license required; strong time management and autonomous

Qualifications

• Must hold a valid Florida RN license and be in good standing.
• CPR certification is required.

Responsibilities

Job Summary

The on-call RN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed. Located in Broward area.

Responsibilities and Duties

On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.

Qualifications and Skills

• Graduate of an accredited school of nursing and current licensure by FL State Board of Nursing
• Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful.
• CPR certification required.
• Holds valid driver's license and current drivers insurance coverage.
