Halifax Health is seeking a Registered Nurse for hospice care in Florida. The RN coordinates and implements the overall plan of care to ensure quality of life for terminally ill patients in various hospice settings. A valid Florida RN license and associate degree are required; bachelor’s degree is a plus.

Qualifications

• Must hold an RN license in Florida.
• Associate degree from an accredited School of Nursing required; bachelor's degree a plus.
• Valid driving license and good driving record.
• Professional interpersonal communication with patients, families and staff.

Responsabilités

• Coordinate and implement the plan of care for hospice patients.
• Assist in after hours and weekend coverage as assigned.
• Perform initial and ongoing assessments and pain management.
• Develop an Interdisciplinary Plan of Care reflecting patient/family goals.
• Participate in quality improvement activities and maintain professional relationships.
