Job Responsibilities

The Home Health & Hospice RN (HHHRN) provides care to patients in their homes, considering the needs of the patient, family, and caregivers. The practice involves a holistic approach to health, from prenatal to post-death periods, and includes primary, secondary, and tertiary prevention. The RN assists patients and families in coordinating community resources and health insurance benefits, along with delivering nursing services in the patient’s home.

Major Responsibilities
  1. Coordinate cases and provide direct nursing care to patients and families on a 7-day basis, including day, evening, night, weekend, and holiday shifts.
  2. Meet patient care productivity standards.
  3. Collect comprehensive data relevant to the patient’s health.
  4. Analyze assessment data to determine diagnoses or issues.
  5. Identify expected outcomes and develop individualized care plans.
  6. Implement care plans and coordinate care delivery.
  7. Employ strategies to promote health, comfort, and safety.
  8. Evaluate progress and improve nursing practice quality.
  9. Maintain current knowledge and competency in nursing practice.
  10. Evaluate own practice against standards and regulations.
  11. Collaborate with colleagues and contribute to professional development.
  12. Integrate ethical principles and evidence-based practices.
  13. Consider safety, effectiveness, and impact in planning and delivering care.
  14. Provide leadership within the practice setting.
Position Qualifications
License/Certification/Education
  • Required: Diploma or Associate’s Degree in Nursing, valid RN licensure in MA, valid driver’s license, access to reliable insured vehicle.
  • Preferred: Bachelor’s, Master’s, or Doctorate in Nursing; specialty certifications; Certified Oasis Specialist.
Experience/Skills
  • Required: 2-5 years recent medical-surgical experience, ability to perform assessments, proficiency with MS Office, ability to work autonomously and communicate effectively.
  • Preferred: Previous case or program management experience.
