Position Details

• Exemption Status: Non-Exempt
• Schedule: Monday through Friday, Holidays (One Major Summer Holiday, One Major Winter Holiday), On Call - Required, Weekends - One Full Weekend per Month
• Hours: 8am-4:30pm (Day Shift, 8 Hours)
• Hours per Week: 32
• Cost Center: 24100 - 4333 Hospice Nursing
• Union: MNA-HealthAlliance Home Health & Hospice (Massachusetts Nurses Association)

Overview

Everyone is a caregiver at UMass Memorial Health. We provide comprehensive home health and hospice nursing care, considering the needs of patients, families, and caregivers across the lifespan. Our approach is holistic, focusing on prevention, education, and hands-on nursing in patients' homes.

Major Responsibilities

1. Coordinate cases and provide direct nursing care 7 days a week, including holidays and weekends.
2. Meet productivity standards and perform comprehensive assessments.
3. Develop and implement individualized care plans, coordinate care delivery, and evaluate patient outcomes.
4. Promote health, safety, and comfort, and contribute to quality improvement and professional development.
5. Collaborate with patients, families, and colleagues, and incorporate ethical principles and evidence-based practices into care.
6. Provide leadership within the professional practice setting.

Qualifications

Required

• Diploma or Associate’s Degree in Nursing
• Valid RN license in Massachusetts
• Valid driver’s license and access to reliable transportation
• 2-5 years recent medical-surgical or home health/hospice experience
• Proficiency with computers and clinical software
• Ability to perform assessments, work autonomously, and communicate effectively

Preferred

• Bachelor’s, Master’s, or Doctorate in Nursing
• Specialty certifications (e.g., hospice, palliative care)
• Experience in case or program management
