Job summary

Heartland Hospice, part of ProMedica, is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families.

Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation.

Qualifications

• RN license required and CPR certification preferred.
• Experience in hospice or end-of-life care is a plus.
• Strong communication and empathy for patients and families.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Coordinate with the interdisciplinary team to deliver holistic care.
• Educate patients and families on disease, treatment options, and home care practices.
• Document care activities timely and accurately in medical records.
• Participate in on-call rotations and collaborate with physicians and social workers.
