Job summary

Qualifications

• Must hold an active Registered Nurse license.
• Hospice or end-of-life care experience preferred.
• Ability to provide care in home and facility settings.

Responsibilities

• Perform comprehensive nursing assessments and develop care plans.
• Deliver skilled nursing and symptom/pain management.
• Coordinate with the interdisciplinary team to meet patient needs.
• Educate patients and families about care and expectations.
• Document timely and accurately in patient records.
• Participate in on-call rotation to ensure 24/7 coverage.
