Job summary

Heartland Hospice is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings.

The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families. Responsibilities include assessments, individualized care planning, education, and timely documentation, with participation in on-call rotations and collaboration with the care team.

Qualifications

• Registered Nurse (RN) with hospice care orientation.
• Ability to assess and respond to symptom needs.
• Plan and coordinate patient-centered care in home or facility settings.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Coordinate with interdisciplinary team to ensure seamless care.
• Educate patients and families on care options, medications, and procedures.
• Maintain timely and accurate documentation of care provided.
• Participate in on-call rotations and remote support as needed.
• Collaborate with physicians and social workers to uphold dignity and comfort at end of life.
