Job Responsibilities

• The Hospice RN provides skilled nursing assessments of clients and caregivers in the Homecare setting, including physical, psychosocial, and environmental assessments and care.
• Develops a care plan in consultation with the client/family, interdisciplinary team, and primary physician.
• Coordinates and plans with involved professionals and caregivers to meet the client’s care goals.
• Supervises LPNs and HHAs.

Work Schedule and Qualifications

• Schedule: 32 hours/month plus weekend and holiday rotations.
• Qualifications: Associate’s degree in nursing, MN or WI RN license, valid driver’s license, reliable vehicle, and ability to work independently in home settings.

• Bachelor’s or Master’s in Nursing, experience in Homecare, Hospice, or Long-Term Care.
