Job summary

The Hospice Homecare Case Manager Registered Nurse provides compassionate, patient-centered care in a home setting, coordinating services to support patients and their families through end-of-life care.

Responsibilities include assessment, care planning, coordinating with multidisciplinary teams, educating families on disease management, monitoring progress, and maintaining accurate documentation within regulatory guidelines.

Qualifications

• RN license required.
• Associate Degree in Nursing or higher.
• Minimum of 1 year of relevant experience.
• BCLS certification within 30 days of hire.
• Valid Driver License within 30 days of employment.
• Strong communication, organizational, and compassionate skills.
