Job summary

Heartland Hospice, part of ProMedica, is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families.

Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation.

Qualifications

• RNs with hospice care experience preferred.
• Strong communication and empathy.
• Ability to provide care in home and facility settings while coordinating with the care team.

Responsibilities

• Provide skilled nursing, pain and symptom management for patients with life-limiting illnesses.
• Develop individualized care plans based on assessments.
• Coordinate with the interdisciplinary team to ensure comprehensive care.
• Educate patients and families about condition, goals, and care options.
• Document care accurately and timely in the medical record.
• Participate in on-call rotations and after-hours coverage.
• Collaborate with physicians and social workers to support patient dignity and comfort.
