Job summary

Heartland Hospice is seeking a compassionate Registered Nurse to deliver hospice care in home and facility settings. The RN provides skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families.

Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation.

Qualifications

• Licensed in nursing and eligible to practice in Iowa.
• Demonstrated compassion in hospice care.
• Experience in pain and symptom management for life-limited illnesses.

Responsibilities

• Provide hospice care in home and facility settings.
• Perform comprehensive patient assessments and develop care plans.
• Coordinate with interdisciplinary team and educate patients and families.
• Document timely and accurately and participate in on-call rotation.
• Collaborate with physicians and social workers to support end-of-life goals.
