Amedisys is seeking a Registered Nurse to deliver in-home care in Shelby County, TN. You will perform patient assessments, develop and monitor care plans, and collaborate with the care team to ensure high-quality, in-home hospice and home health services.

You will educate families, document progress, and participate in on-call coverage as needed. This role requires current licensure and CPR certification, with preferred 1 year of nursing experience.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• One year of nursing experience preferred; new grads accepted into RN program.
• Current CPR certification.

Responsabilités

• Performs initial nursing assessment on new admissions.
• Completes and revises the plan of care with the care team.
• Communicates patient progress to clinical manager and physicians.
• Maintains patient care records with accuracy and timeliness.
• Records pain/symptom changes and outcomes.
• Educates families on patient condition and care.
• Makes referrals to other disciplines as needed or documents rationale.
• On-call responsibilities and on-call services as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
