Cheer Home Care, LLC is actively hiring a Private Duty Registered Nurse for per diem, offering hourly pay based on experience in the San Diego area. The RN will deliver skilled nursing care in patients' homes, collaborate with an interdisciplinary team, and educate families while documenting care in EHR systems.

Requirements include an active California RN license, 3 years RN experience (home health preferred), TB and health screenings, a valid CA driver’s license, and reliable internet-enabled

Qualificações

• Current, unrestricted California RN license.
• 3 years of RN experience.
• Home Health experience preferred.
• Proof of negative TB result.
• Physical screening documentation within the last year.
• Active California driver’s license and car insurance.
• Cell phone with Internet access.

Responsabilidades

• Provide skilled nursing care including wound care, IV therapy, and monitoring of chronic/acute conditions.
• Manage patients with complex needs (ventilators, tracheostomies, feeding tubes).
• Recognize early signs of deterioration and intervene promptly.
• Educate patients and families on disease management, medications, and equipment use.
• Coordinate care with physicians, therapists, and other healthcare providers.
• Maintain accurate, timely documentation in electronic health records.
• Ensure compliance with home health regulations and infection control practices.

Hourly Pay Rate: $45-$80 based on experience

Shifts: 12 Hour Day Shifts

Cheer Home Care is looking for a Registered Nurse who loves spending time with older adults and provide high quality care in home. The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. .

This position responsibilities will include:

• Provide skilled nursing care, including wound care, medication administration, IV therapy, and monitoring of chronic/acute conditions
• Manage and monitor patients with complex needs (e.g., ventilators, tracheostomies, feeding tubes)
• Recognize early signs of deterioration and intervene appropriately
• Educate patients and family members on disease management, medications, and equipment use
• Coordinate care with physicians, therapists, and other healthcare providers
• Maintain accurate and timely documentation in electronic health records
