Cheer Home Care, LLC is actively seeking a Private Duty Registered Nurse for per diem shifts in San Diego, CA. The RN will provide in-home skilled nursing, collaborate with an interdisciplinary team, and support individualized care plans for older adults.

The ideal candidate holds a CA RN license, has 3 years of RN experience (home health preferred), and can manage complex conditions with proper documentation and patient education.

Qualificações

• Current, unrestricted California registered nurse license.
• 3 years of RN experience.
• Home Health experience preferred.
• Proof of Negative TB result, yearly physical screening.

Responsabilidades

• Provide skilled nursing care including wound care and medications.
• Manage patients with complex needs (ventilators, tracheostomies, feeding tubes).
• Monitor for deterioration and intervene appropriately.
• Educate patients and families on disease management and medications.
• Coordinate care with physicians and other health professionals.
• Maintain accurate documentation in electronic health records.
• Ensure compliance with home health regulations and infection control.

Hourly Pay Rate: $45-$80 based on experience

Shifts: 12 Hour NOC Shifts

Cheer Home Care is looking for a Registered Nurse who loves spending time with older adults and provide high quality care in home. The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. .

This position responsibilities will include:

• Provide skilled nursing care, including wound care, medication administration, IV therapy, and monitoring of chronic/acute conditions
• Manage and monitor patients with complex needs (e.g., ventilators, tracheostomies, feeding tubes)
• Recognize early signs of deterioration and intervene appropriately
• Educate patients and family members on disease management, medications, and equipment use
• Coordinate care with physicians, therapists, and other healthcare providers
• Maintain accurate and timely documentation in electronic health records
• Ensure compliance with home health regulations and infection control practices

• Current, unrestricted California registered nurse license

• 3 years of experience as a registered nurse
• Home Health experience preferred
• Proof of Negative TB result
